Kindersley Health Services Needs Assessment: Moving Forward

Thursday night we had a good turnout for the release of the Kindersley Health Services Needs Assessment. Lawrence Beaudry, the lead consultant with WMC, did a terrific presentation. You can find the presentation here.

An implementation plan has been developed. The core of the implementation plan is what is currently termed as the 'Community Advisory Network.' (CAN) The purpose of the network, once it is fully operating, is to provide oversight to the implementation of the recommendations and provide strategic input.

It is Councillor McTaggart's and mine hope that the Town can step away from health care in 9 months or assured and confident that CAN is effectively overseeing the implementation of the recommendations, ensuring the stability and sustainability of the delivery of health services in Kindersley, and providing vision and strategic input.

Potential models include Meadow Lake, Cudworth, and Wakaw.

Community Advisory Network: Wakaw

Purpose

The Collaborative Emergency Centre, (CEC) Development Group is established to:

·         Work collaboratively with the communities within Wakaw/Cudworth area and the Saskatoon Health Region in the establishment, implementation and operation of a Collaborative Emergency Centre for the Wakaw/Cudworth area.

·         Facilitate communication with the stakeholders within the Health Region and communities involved.

·         Assist in gathering information regarding community needs and expectations regarding CEC development.

·         Assist in developing the CEC model, policies and procedures to ensure access to safe, effective and efficient assessment and treatment of patients.

Accountability

This committee will report to the Vice President of Integrated Health Services and to the Community Leaders.

Membership

Specific Accountabilities

1.       Provides a forum for education, discussion, sharing, problem solving and decision making.

2.       This committee will forward recommendations to the Vice President of Integrated Health Services (Cory Miller) and the Saskatchewan Ministry of Health regarding the model of CEC that will best meet the needs of the community.

Commitment

·         This is a “CEC Development Committee” designed to create a model of CEC to be established to meet the needs of the community.

·         Meeting frequency will be as required. We anticipate bi-weekly or tri-weekly meetings.

·         Quorum: a majority of members present with “fair representation” of the committee stakeholders.

·         Decisions will be made by “consensus” defined by the following:

·         there has been vigorous debate on the subject by the group wherein a large range of points of view has been presented ranging from absolute support to absolute negation and everyone has had an opportunity to voice their view. (The exception to this shall apply in cases of unanimity or near-unanimity)

·         once the issue has been analyzed and debated, a “check” will take place to see if a large percentage of persons involved in the discussion are in support or opposition of the idea. This is not a vote but a “voicing of opinion” supported by the views of others in the debate.

·         the chair will decide if there is a wide enough margin that the following conditions would be met:

·         The decision would not be quickly reversible due to an outcry over the margin by which consensus was gained

·         The decision would not be reversible due to general dislike of the idea itself

·         The chair will consider the magnitude, scope and consequentiality of the idea and decide if there is consensus.

·         Submit agenda items one week prior to the meeting

·         Agendas and meeting minutes will be forwarded to the Vice President of Integrated Health Services

·         Responsibility for minutes belongs to the chairs (admin support) and will be circulated via email

·         Review Terms of Reference at first meeting

Community Advisory Network: Cudworth

Purpose

To promote the active engagement of individuals  and communities in planning.re­ viewing and Implementing strategies that both maintain and improve the health and well being of citizens 1n the Saskatoon Health Region.

1. To develop processes that support communities as active partners in their health and health care  services.

2. To partner with communities and key stakeholders to jointly share information and promote understanding of perspectives that might not otherwise be under stood.

3. To build capacity through 1nforming and empowering Individuals and communities to take more responsibility and control over their health.

4. To build mutual trust and credibility through the process of engagement.

5. To promote effective, efficient and sustainable services that appropriately meet the needs  of communities

The Terms of Engagement will be reviewed on an annual basis.

Guiding Principles

1. Communities will be actively and meaningfully engaged in identifying community needs and participating in key strategic initiatives.

2. Except In circumstances beyond the control of the Saskatoon Regional Health Authority no decisions shall be made  to eliminate, significantly change or add to existing services without the knowledge  of, active Input from and discussion of options with the local Community Health Council.

It is understood the Saskatoon Regional Health Authority has the ultimate

final decision making authority.

3. SHR and the local communities will openly share information that affects the health and well being of residents.

4. SHR and the local communities will support healthy living by participating together  to address priority health promotion and disease prevention strategies.

5. The engagement process must be transparent, legitimate and official. Community input is valued and the region is committed to providing feedback about its decisions.

6. Participants should be involved as early as possible in the process.

7. The potential for communities to influence decisions must be real.

8. Information must be timely such that sufficient time is allowed to under­ stand, discuss and deliberate. Information must be clear, transparent and easy to understand.

9. SHR resources must be available to support engagement processes.

10.SHR and the Council shall strive to achieve mutually acceptable out­comes.

Community Advisory Network: Meadow Lake

Purpose:

This group was formed to promote the development of Primary Health Care in Meadow Lake and area in partnership with the Meadow Lake Associate Clinic. The Primary Health Care Leadership Team will provide a forum for communication, creative problem solving and collaborative visioning for the development of the Primary Health Care services in Meadow Lake and surrounding community.

Membership

Membership is open to health providers within PNRHA, First Nation agencies and other interested parties. Membership includes:

·         Clinic Manager

·         Primary Health Care Counselor

·         Physician Rep.                                             

·         Primary Health Care Team Facilitator

·         Meadow Lake Tribal Council representative

·         Primary Health Care Team  Manager

·         Director of Care at Meadow Lake Hospital

·         CHAN representatives

·         Population Health Services Rep.

·         R.M. of Meadow Lake  Rep.

·         City of Meadow Lake  Rep.

·         Director of Primary Health Services

·         Manager of Ambulatory Care/EMS

·         Homecare Rep.

Mandate:

1. To develop a common vision and direction for Primary Health Care Service development.

2. To create an awareness of services provided, as well as new initiatives and progress that are being developed in order to maximize efforts and avoid duplication.

3. To support the development of Primary Health Care Services by soliciting and sharing advice, developing coordinated, cooperative solutions to issues as well as to creatively manage and solve problems.

4. To participate in the review and make recommendations of Primary Health Care services through the review of team functioning with the Team Effectiveness Tool, Clinical Practice Redesign, The Patient Survey and other data as relevant.

5. To support and promote team work and positive working relationships with the community.

6. The Terms of Reference will be reviewed annually. 

Meetings

1. Will be held monthly or as called by the group.
2. Minutes of all meetings will be shared with the membership.
3. The co-chair persons will be the Primary Health Care Team Facilitator and the clinic manager.
4. The chair person will circulate an agenda prior to each meeting.
5. The recorder is responsible to document the meeting and distribute the minutes to the team facilitator who will distribute to all members of the Primary Health Care Team. 
6. The hosting agency will donate the meeting room.
7. Travel to meetings and related expenses are the responsibility of each member’s employer.
